Everyone knows Yard House is famous for its great atmosphere and made from scratch dishes. However, the restaurant is likely far less known for its to-go service. Nonetheless, it appears Yard House wants to know how it’s doing in this area. Fortunately, customers have an easy way of helping out. In order to launch the YH To Go Survey, customers must first enter the ID number shown on their receipt. Once the customer enters the number they can click the Start button to proceed with the feedback. Upon completion of the survey customers can enter a sweepstakes for a chance to win $1,000.

For instance, patients can now take a brief online survey about a recent audiologist visit in exchange for entry to a Prize Draw. In order to begin the survey, patients must first enter the doctor’s name, date of the visit, and time of the visit. Once the patient enters all of the required information they can click the Start button to proceed. Upon successful completion of the survey, the participant will receive an invitation to enter the Prize Draw.

In order to get started with The Range Store Feedback survey, customers must first confirm whether or not they have a receipt. Customers who have a receipt must enter the 20 digit code (located at the bottom of the receipt) along with the total amount spent and the name of the person who provided service. Meanwhile, those who DO NOT have a receipt can skip straight to the part where they enter the name of the store.
